Making Choices Information Card

This easy-to-read Making Choices® Information Card is helpful to patients upon admission. It offers a brief explanation about advance care planning and indicates who the patient should contact for more information. The Card helps meet regulations established by the Patient Self-determination Act of 1991 and can be customized for your institution. Card measures 5-1/2 x 10-1/4.
