Billing Policy
For orders to be invoiced, we require a minimum order (including shipping & handling) of $100.
For orders totaling less than $100 (including shipping & handling), please prepay by check or credit card. These orders are processed when we receive your payment.
If you use a credit card to place your order, your credit card statement will read Gundersen Lutheran Medical Foundation.
Return Policy
We create high quality materials to send a message of support to patients engaged in advance care planning. Our materials require special handling to prevent damage during shipping. If you receive damaged items, please call within seven days and we will gladly replace the items.
We are unable to accept returns for any other reason.
